The Partnership of Healing: A Plastic Surgeon’s Guide to Post-Operative Care

When patients walk into my consultation room, they often view surgery as the main transformational event, a single moment where their lives are changed forever. They imagine that once they leave the operating theatre, the journey is essentially complete.

In reality, surgery is only the start. While surgical technique and precision account for roughly 80% of your outcome, the remaining 20% rests entirely on what happens next. The post-operative phase isn't just a period of waiting; it is a collaborative partnership between you and your clinical team.

Understanding the Tissue Journey

The perfect outcome requires time, respect for human anatomy and to the subsequent healing process. While skin incisions typically heal within 7 to 10 days, complete tissue remodelling and scar maturation may take up to 18 months or even two years.

During the "ugly phase" of healing, scars can appear red, raised, or lumpy. This is a completely natural part of the biological process of healing. Once your wounds have fully healed, a gentle twice a day massage with a moisturising cream helps scars to mature, flatten out, fade in colour, become soft and eventually less noticeable.

Different skin types heal uniquely, and if a scar develops stubbornly, we intervene early with pressure therapy or steroid injections into the scar to flatten the tissue. Patience during this phase is essential.

Protocol Is for Protection, Not Just Advice

The single biggest mistake a patient can make during recovery is breaking post operative instructions or protocols. Protocols such as wearing prescribed compression garments, sleeping upright with head elevated, avoiding heavy lifting or strenuous exercise are designed to protect fragile internal tissues while healing.

Exerting yourself too early or neglecting post-op guidance increases internal pressure and strain on incision lines. This can lead to:

  • Wound breakdown or delayed healing

  • Infection or fluid accumulation

  • Wider, thicker scarring that compromises your final aesthetic result

Surgeons establish these rules not to restrict your lifestyle, but to safeguard your results. If I could give every patient a single golden rule for a smooth recovery, it would be this: communicate with us, as Ria did. Occasionally you may not be able to get in touch with the team immediately; the advice would be to call again after some time or leave a message on the phone or Whatsapp requesting a call back. The team will endeavour to return your call as early as possible.
